INVITATIONAL ONLY! Do not make a booth payment unless you have received an official invitation email from the PRTE as payments are non-refundable!

Artist Fee: Each individual tattoo and permanent makeup artists or body piercing technicians must submit a Health Permit Artist Application and pay the appropriate permit fees INCLUDED IN YOUR BOOTH PAYMENT. All Artist and Event Coordinator Applications must be submitted with ALL required documentation via email to bodyart@snhd.org or fax to (702) 759-1486.
After the application has been submitted and processed, an invoice will be forwarded to you. All invoices must be paid with seven days of receipt. Failure to pay the fees may result in denial of the permit application.
ARTISTS MUST PROVIDE:
- Proof of licensing, a photo of your licensing.
- Proof of Bloodborne Pathogens certification, you must provide a photo of your certificate:
ALL APPLICATIONS MUST BE SUBMITTED A MINIMUM OF 30 DAYS PRIOR TO THE EVENT. ALL PERMIT FEES ARE NON-REFUNDABLE AND NON-TRANSFERABLE. NO EXCEPTIONS.
